Maestro Overheating

A bit of advice please.....The car was overheating and there was a puddle of water on the floor. I traced it back to the top up bottle on the seam there was a pinhole leak :( . I have replaced the expansion bottle.:)

First question, what would have caused the pressure to be so high to have found the weakness in the bottle or is it just age.

Second What level should the temp gauge reach before the fan comes on, mine os just over a 1/4

Third question Should the hoses feel pressurised.

Fourth question What PSI rating should the radiator cap be mine is 15PSI. Do these ever go wrong ? The seal looks reasonable

Fifth qustion and unrelated, Does anyone know where I can get those rubber elbows and t's for the vacuum hoses, no where seems to sell them?

I got an elbow with short hose from a car shop - it was designated for something else but I took the elbow off and used that. And made up a T piece to fit using a wrong size T that was meant for washer hoses and added bits of fuel pipe each side. There's a thread about it somewhere I think but I can't find it.
